Referrals
Families of
young children with special needs such as
Attention Deficit/ Hyperactivity Disorder
(AD/HD), Reactive Attachment Disorder (RAD), or
Autism Spectrum Disorder (ASD) may be
overwhelmed and find it hard to access or follow
through with services needed to ensure school
readiness for their child.
These
children may fall through gaps in educational
services or be overlooked by agencies/programs .
For some families, one incorrect or
misunderstood piece of information may be enough
to derail them from following through with
telephone calls, scheduled meetings, or
paperwork needed to secure services for their
child and family.
Bridging the Gap
offers assistance to these families through
intensive, short-term case management.
Information about possible accommodations, the
special education system, and other available
services is provided. Hands-on assistance with
setting up, attending, and participation in
meetings is provided. Families also learn about
other services available to assist the entire
family.
The goal of the
Bridging the Gap
program is for young children who have special
needs to enter kindergarten smoothly and
successfully with all partners and the family
supplied with the knowledge and support services
needed to ensure school success. |
